Completing the form is easy. Click in the Parent/Guardian Name field, type your full name, then tab to the successive fields.
1)	Type your data into the form.
2)	Print the document.
3)	SIGN THE RELEASE FORM.
4)	Bring the Release Form on your first day of classes and present to the office OR
	mail it to the following address:
		McCracken's Gymnastics & Swim School
		15515 S. Keeler St.
		Olathe, KS 66062
Note:	Participation in McCracken's Gymnastics and Swim School programs requires that the Parent/Guardian of every chid who participates in any activity at McCracken's complete and sign the Release Form for those minors. Students will not be allowed to participate until the form is on file at our office.
